The Intake Specialist obtains patient information via telephone interviews with patients, their families or other referral sources.
Consults with clinical staff to provide a provisional level of care (LOC) recommendations and schedules patients for that LOC evaluation.
Enters intake information into the computer system and makes every effort to provide the patient with a smooth and efficient entry into Carrier Clinic.
Responsibilities:
Answers all telephone inquiries promptly and provides pertinent information regarding inpatient services to patients, families and referral sources.
Enters accurate and complete patient data (demographics, insurance, and clinical information) into the computer system.
Triages patients for level of care in consultation with clinical staff.
Schedules patient appointments and provides assistance in all aspects of patient intake.
Cross-trained to perform registration, financial counseling, and other tasks when needed.
Requests medical records and other clinical information from professional referral sources prior to patient's evaluation Initiates patient call backs.
Provides reassurance and support to the patients, families and friends in order to reduce anxiety levels associated with the admissions process.
Serves on hospital committees as appointed.
Follow up with patients to complete paperwork not signed during the admissions process.
Other duties and/or projects as assigned.
Adheres to HMH Organizational competencies and standards of behavior.
Qualifications:
High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
Excellent written and verbal communication skills.
Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
Associate's or Bachelor's degree from an accredited college/ university in social work, psychology, nursing or related field.
One year behavioral health care experience.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
